SIDE BY SIDE is LCC Services’ person-centred transition support program for young people and young adults moving from custody back into the community. We understand that release is more than simply leaving custody. It means rebuilding routines, reconnecting with family and community, attending important appointments, developing independence and making positive choices about what comes next.
Where the young person has appropriate NDIS funding, our first step is to understand their individual plan, funded supports, goals and support needs. From there, we work alongside the participant, their family and relevant support network to develop practical supports that align with their NDIS goals and complement existing justice, health and community services.
How we Support
One-to-One Transition Support
Where possible, we aim to meet the young person before release. This allows us to begin building trust, understand their goals and identify what support may be important as they return to community.
Following release, an experienced Youth Support Worker can provide consistent, practical support as the participant establishes themselves back in community.
Support may include:
• Building everyday routines and independent living skills
• Accessing identification, banking and essential services
• Reconnecting with education, training or employment
• Attending health, allied health and other relevant appointments
• Connecting with sporting, cultural and community activities
• Building social skills, confidence and positive relationships
• Practical mentoring and positive role modelling
Where appropriate and within funded supports, we can also assist participants to attend Youth Justice, police, court, bail-related or other required appointments. Our role is to support engagement and attendance and does not replace statutory or clinical services.

SIDE BY SIDE works collaboratively with participants, families, Support Coordinators, Youth Justice, health professionals, education providers and community organisations to support a coordinated transition back into community.
